The part that gets missed
Testosterone doesn't move alone.
Some of the testosterone in your body is converted into estradiol by an enzyme called aromatase. That's normal and necessary — men need estradiol. But when testosterone rises, conversion can rise with it, and for some men that ratio is what's driving the symptoms rather than the testosterone itself. It is measurable, which means it is manageable.

Estradiol is routinely left unmeasured in men, so a conversion problem gets read as a testosterone problem and treated with more testosterone.
